• Your real first and last name
• Your telephone number/contact information
• A secondary telephone number in case you are not at the one you give
• Your e-mail address
• A description of the lost pet
• Note:  Hold back some small bit of information to use for positive identification during a conversation where you can not see the pet that is suspected of being yours.  For example, a different patch of fur someplace easily visible, a fat pocket, anything easily seen up close but not so obvious from a distance. 
• A color photograph of the lost pet. 
• Reward amount, IF being offered
